Introducing the NEW Concussion Passport...
Every concussion is as individual as the person who has it. Which means concussion care is not a one-size-fits-all endeavour.
The most effective concussion recovery plan is customized for each patient and includes treatment for and tracking of cognitive, physical, vestibular and vision symptoms.
Most concussion patients don't benefit from this type of integrated care because they receive treatment from different health care providers who don't communicate with each other because they are working in different locations.
The Concussion Passport has been designed to put concussion management in the hands of the person who cares about it most — the patient.
The Passport is a treatment tool that tracks a patient's journey from injury to recovery and return to school, work and play.
It gives the patient something tangible to share with their family, teachers, coaches, employers —and their various health care providers — to communicate the treatment they've received and where they are on their concussion recovery journey.
The Concussion Passport will be available for purchase in Fall 2016.
